SUPERVIVENCIA SEGÚN ESTADIO INICIAL
CÁNCER DE TIROIDES. ANATOMÍA PATOLÓGICA
CÁNCER DE TIROIDES. TRATAMIENTO
� Alto riesgo: (edad > 45 a., hombre, metástasis, afectación extratiroidal, > 4 cm)� Tiroidectomía total + linfadenectomía
� RAI (I131) ablación� Supresión TSH (hormona tiroidea)
� Seguimiento niveles tiroglobulina sérica
� CRT para recurrencias locales / márgenes positivos
� Enfermedad refractaria a radioyodo y enfermedad metastásica:� No cirugía ni ablación
CÁNCER DIFERENCIADO DE TIROIDES YODO REFRACTARIO
� 5 – 15 % de los pacientes
� Supervivencia media de 2.5 – 3.5 años (en caso de presencia de metástasis)
� No existe tratamiento estándar para el cáncer diferenciado de tiroides yodo refractario.
MUTACIONES EN EL CDT.
� Mutaciones en BRAF, RAS y reordenamientos de RET están implicados en más del 70 % de los CDT.� PAPILAR:
� RAF 45 %� RAS 15 %� RET/PTC 20 %� C-MET 80 %
� FOLICULAR:� PI3K/Akt 80 %� RAS 45 %� PAX8/PPARgamma 20 %
� Dianas terapéuticas:� Angiogénesis (todos los subtipos)� AMPK y vía PI3K/AKT mTOR (derivados de células foliculares)
