network. If your primary care physician (PCP) says you need services outside of our network, s/he

can call us. Refer to your member handbook if you need help.

use of referrals and other services you might need. To get this information, call Member Services at

for you and your family is easy.

There are several types of PCPs listed here.

To find a doctor, choose the county you want in the table of contents. Choose the Specialty you want,

such as Primary Care Providers. (Specialty is the area of medicine in which this doctor is highly

trained. The doctor trains in this area of medicine after medical school.) Remember to take your

member ID card with you.

These listings are based on the provider's application. We verify certification and hospital affiliations when

a provider joins our network. We check every three years (or sooner). We verify names, phone numbers,

addresses, languages, limits and gender when a provider joins our network or gives us new information.

TABLE/SCALE

The exam table moves up and down and the scale is accessible with handrails

to assist people with wheelchairs and scooters. The weight scale is able to

accommodate a wheelchair.
