• PEMBROLIZUMAB EN MONOTERAPIA: PD-L1 se posiciona como el únicobiomarcador que puede ayudarnos a seleccionar pacientes.– Claro en población de >20 PERO… Y LAS RESPUESTAS? … AUNQUE SON DURADERAS
• PEMBROLIZUMAB + quimioterapia es el nuevo estándar sin selección por PD-L1
-Pero….. Siempre?
-Y en PD-L1 ENTRE 1% Y 20%?
-Y EN PD-L1 >1% ES MEJOR PEMBRO SOLO O PEMBRO-QT?
• Queda lugar para platino + cetuximab en primera línea?
REFLEXIONES DE LA PRIMERA LÍNEA
• La inmunoterapia ha irrumpido en el escenario de la enfermedad metastásica comouna nueva opción en segunda y posteriores líneas y recientemente en la primeralínea.
• No sabemos seleccionar a priori los pacientes que van a presentar respuesta, locual es factor de aumento de supervivencia, aunque PD-L1 se posiciona como elúnico biomarcador que puede ayudarnos a seleccionar pacientes.
• La quimioterapia basada en platino y en combinación con cetuximab sigue siendoEL ESTÁNDAR en primera línea AUNQUE LOS NUEVOS RESULTADOS VAN ACAMBIAR DICHO ESTÁNDAR.
• La PFS y la rapidez de respuesta sigue siendo un parámetro a tener en cuenta a lahora de elegir quimioterapia o inmunoterapia en según que situaciones.
