FISIOPATOGENIA DE LA EPOC
Fisiopatogenia de la EPOC
El mayor sitio de obstrucción en la EPOC está a nivel de la PVA (bronquiolos < 2mm)
La limitación al flujo aéreo es principalmente irreversible dado que la obstrucción se debe no sólo a la hipersecreción de moco, sino a la fibrosis perialveolar y a la pérdida de alvéolos.
La limitación al flujo aéreo es progresiva y está asociada a una respuesta inflamatoria anormal del pulmón a diferentes noxas o gases, especialmente el tabaco.

Exacerbaciones de la EPOC
Las exacerbaciones de la EPOC son episodios clínicos habituales en su historia natural.
Empeoramiento mantenido del paciente en su situación basal de disnea, tos y/o expectoración, más allá de la variabilidad diaria, que requiere un cambio del tratamiento habitual.

– Empeoramiento en el estado de salud y la calidad de vida
– Visitas frecuentes al médico de familia, servicios de urgencias e ingresos hospitalarios

El 75% de las EAEPOC son de causa infecciosa, de ellas en las tres cuartas partes, la etiología es bacteriana y en el resto la causa es fundamentalmente viral

Tratamiento 3 meses con OM-85 BV (estudio doble ciego, randomizado, placebo controlado [190/191]
Conclusiones
El riesgo de presentar al menos un episodio de EAEPOC (objetivo primario) fue similar en los dos grupos (p=0,872)
El grupo de tratamiento activo, tuvo EA de menor severidad y el riesgo de ser hospitalizado fue menor (-29,9%).
El tiempo de duración de la hospitalización fue < en el grupo activo (OM-85: 1,5 días vs placebo: 3,4 días).
La disnea fue menos marcada durante las EA. Muertes: OM-85: 2 vs placebo: 6. Dado que el efecto protector del OM-85 BV estaría
mediado a través del la estimulación del SI, le da a este tratamiento una potencialidad interesante como complemento del tratamiento regular de la EPOC.

• Afectan a toda la población, > a los menores de 5 años y a las personas de 65 años
• En Argentina– Las infecciones respiratorias agudas (IRA)
representan la primera causa de consulta a los servicios de salud.
– La principal causa de consulta e internación es la enfermedad respiratoria en todas las edades.
– El 60% de los niños menores de 1 año y el 50% de los menores de 5 años, padecen un episodio de IRA en el trascurso de un año.

regulatory T cells to the airways. Bacterial extracts are currently used in children suffering from
repeated upper respiratory tract infections.
this study, the authors investigated whether bacterial extracts, commercially available as Broncho-Vaxom (BV), could prevent allergic airway disease in mice.
Oral treatment with BV suppressed airway inflammation through IL-10-dependent mechanisms and induced the conversion of FoxP3-negative T cells into FoxP3-positive regulatory T cells.
Furthermore, CD4-positive T cells purified from the trachea of BV-treated mice conferred protection against airway inflammation when adoptively transferred into sensitized mice
Conclusion
Treatment with BV could possibly be a safe and efficient strategy to prevent the development of allergic disease in children.

Conclusion
