La imagen confirmatoria
Hace exactamente 4 años que nuestra revista nació apelando a la curiosidad,
siendo esta un pilar necesario de la investigación y como denominador común de
los grandes científicos que han existido y existen en la historia de la humanidad.
Una revista que empezaba publicando "imágenes curiosas".
Muchas veces creemos que la ciencia es matemática, estadística, etc., pero
realmente son algunas de las herramientas del científico para comprobar sus
hipótesis.

Las imágenes son otras grandes herramientas que además cuentan en muchas ocasiones con cierta
belleza que satisface aún más la curiosidad, a la vez que pueden suponer una flamante confirmación de una
teoría científica. Por poner un ejemplo del año que termina, conseguir una imagen de un agujero negro, ese
densísimo objeto astronómico, ha sido un hito de la ciencia y de la tecnología que ha requerido la
colaboración de más de 200 investigadores repartidos en ocho observatorios espaciales de todo el planeta,
y que confirma los postulados de la Relatividad General de Albert Einstein. Las diferentes sondas que orbitan
Marte, junto con los Rovers Opportunity, Spirit, Curiosity y ahora InSight, nos han reportado amaneceres y
atardeceres marcianos, junto con imágenes, y pruebas químicas también, de agua en estado sólido.
Pero conseguir una imagen importante no tiene por qué requerir ese grandísimo derroche científico y
tecnológico, por ejemplo, en este año también gracias a la colaboración de los indígenas Arhuacos
colombianos, la ciencia ha vuelto a documentar la rana arlequín de la noche estrellada, considerada extinta
durante 30 años.
El ámbito del Laboratorio Médico no es una excepción. La imagen juega un papel importante, lo habéis
demostrado vosotros, nuestros autores y lectores, sois los verdaderos artífices de que sigamos publicando
imágenes científicas, de forma que "de un vistazo", despertemos la curiosidad de los compañeros
profesionales sanitarios.

Calcita es el nombre mineral del carbonato
cálcico (CaCO3), de la que una forma especial de
cristalización es conocida como aragonita. Ésta
constituye un arquetipo cristalino propio que puede
formar cálculos mixtos o de contenido puro. Pese a
ello, no está apenas reflejada en las clasificaciones de
las especies cristalinas presentes en las litiasis
urinarias humanas debido a su inusual aparición. De
hecho, los cálculos urinarios de carbonato cálcico son
actualmente excepcionales y su producción se asocia
a la eliminación crónica de productos farmacológicos,
alcalinizantes gástricos (actualmente muy poco
utilizados), o bien como consecuencia de una
malnutrición extrema. Por el contrario, el carbonato
cálcico es relativamente frecuente en la litiasis biliar.
Las dos variedades de carbonato cálcico que se
describen en este artículo reaccionan desprendiendo
CO2 ante la adición de ácido clorhídrico y se
diferencian en su aspecto, cristalización y estudio
infrarrojo:
La calcita tiene un aspecto similar al mármol
y tiene una tendencia blanda. Cristaliza en
sistema piramidal y se caracteriza por
presentar bandas a 2500, 1700-1300, 875 y
690 cm-1.
La aragonita es dura al corte, frecuentemente
de color blanco, aunque también aparece de
color violáceo, marrón, negro, azul o verde.
Cristaliza en sistema ortorrómbico y destaca
en su espectro infrarrojo una gran banda de
absorción entre 1600-1400 y otra a 875 cm-1.
El examen morfológico se realizó mediante el
corte y observación de los fragmentos a través de
microscopía estereoscópica y su composición se
determinó por medio de espectroscopía infrarroja
(IRAffinity-1S).
Calcite is the mineral name of Calcium
Carbonate (CaCO3), a special form of crystallization is
known as Aragonite. It constitutes an own crystalline
archetype that can form pure or mixed calculi. Despite
this, it is hardly reflected in the classifications of the
crystalline species which can be found in human
urinary lithiasis due to its unusual appearance. In fact,
calcium carbonate urinary stones are currrently
exceptional and their production is associated with the
chronic elimination of gastric alkalizing
pharmacological products (currently they are rarely
used), or as a consequence of extreme malnutrition. In
contrast, calcium carbonate is relatively common in
biliary lithiasis.
The two varieties of calcium carbonate,
previosly descrited, react by emitting CO2 due to the
addition of hydrochloric acid. They could be
differenced by the aspect, the crystallization and
infrared spectrum absorption:
The Calcite looks marble but is soft. It
crystallizes in a piramidal system and is
characterized by presenting bands at 2500,
1700-1300, 875 y 690 cm-1.
The Aragonite is firm when cut it. It is usually
white but it also might be purple, brown, black,
blue or green. It crystallizes in orthorhombic
system and it is outlined by its wide absorption
band between 1600-1400 and another with
875 cm-1
The morfologic study was done by the cut and
the inspection of the fragments by microscope. The
composition was determinated by infrared
spectroscopy (IRAffinity-1S).

La espectroscopía infrarroja (IR) es un método
específico y preciso para la identificación de la
composición química de los cálculos urinarios, a
través de la determinación de la frecuencia a la que,
los distintos grupos funcionales, presentan bandas de

absorción en el rango IR del espectro
electromagnético. La principal ventaja de este método
es la velocidad de análisis, además puede diferenciar
entre un compuesto puro o una mezcla de varios, por
lo que el conocimiento de la técnica de IR y la
experiencia de los especialistas es fundamental para
un análisis correcto.
Una técnica complementaria para detectar el
origen mineral del carbonato es la difracción de rayos
X pero la complejidad y el elevado coste de los
instrumentos imposibilita su aplicación en la práctica
clínica habitual.
Finalmente, ya que el cálculo es el producto
último de un proceso multifactorial, el análisis de su
composición mineral nos permite identificar los
factores determinantes de su formación y obtener un
valor predictivo de las anormalidades metabólicas
subyacentes, así como proponer distintas medidas
higiénico- dietéticas o tratamientos farmacológicos
personalizados que reduzcan el elevado riesgo de
recidivas asociado a esta patología.

composition allows us to identify the determining
factors of its formation and obtain a predictive value of
the underlying metabolic abnormalities, as well as the
different hygienic-dietetic measures or
pharmacological treatments to reduce the high risk of
recurrence associated with this pathology.

Presentamos el caso de una paciente joven,
con antecedentes de infección urinaria con cultivos
positivos, cuya orina es remitida desde Atención
Primaria para su análisis. Entre sus características
físicas, se observa una orina de color pardo oscuro y
olor intenso. Ante los parámetros alterados de la tira
reactiva (pH 9, Proteínas +, Nitritos + y Leucocitos +)
procedimos a la realización del sedimento. En este
destacó la presencia de abundantes formas cristalinas
de morfología inusual, como se muestra en la figura.
Para su estudio e identificación se empleó
microscopía óptica de campo claro, donde vemos que
la estructura del núcleo del cristal es muy similar a la
forma característica en “tapa de ataúd” de los fosfatos
amónico-magnésicos. Además, poseen intensa
birrefringencia ante la luz polarizada (figura).
Esta cristalización atípica corresponde a la
variedad en “hoja de helecho” de los fosfatos amónico-
magnésicos, caracterizada por prolongaciones que
recuerdan a la morfología de la planta.
Su presencia es inusual debido a las concretas
condiciones necesarias para su formación, ya que
casi siempre se trata de una orina contaminada por
bacterias ureolíticas que ha demorado su análisis más
de 8 horas.
Los cristales de fosfato amónico magnésico
(Fosfato triple o estruvita) aparecen
fundamentalmente en orinas alcalinas (pH>8). Son
incoloros, pleomorfos y no aparecen en individuos
sanos, asociándose siempre a infecciones por
bacterias ureolíticas. Si las condiciones
fisicoquímicas, tiempo de permanencia o pH lo
favorecen, estos cristales precipitan en formas
incompletas como trapezoides, formas cruciformes,
prismas o en “hoja de helecho”, como el caso
referenciado.
We present the case of a young female patient,
with a history of urinary tract infection with positive
cultures, whose urine was sent from Primary Care for
analysis. Its physical characteristics included dark
brown urine and intense smell. Given the altered
parameters of the test strip (pH 9, Proteins +, Nitrites
+ and Leukocytes +), we proceeded to make the
sediment analysis. In this one, the presence of
abundant crystalline forms of unusual morphology was
highlighted, as shown in figure.
For its study and identification, bright-field
optical microscopy was used. We observed a structure
of the crystal nucleus very similar to the characteristic
"coffin lid" shape of magnesium ammonium
phosphates. In addition, they had an intense
birefringence against polarized light (figure).
This atypical crystallization corresponds to the
"fern leaf" variety of magnesium ammonium
phosphates, characterized by extensions reminiscent
of the plant morphology.
Its presence is unusual due to the specific
conditions necessary for its formation, since it is
almost always a urine contaminated by ureolitic
bacteria that has delayed its analysis more than 8
hours.
The crystals of magnesium ammonium
phosphate (Triple phosphate or struvite) appear
mainly in alkaline urines (pH>8). They are colourless,
pleomorphic and do not appear in healthy individuals.
They are always associated with infections caused by
ureolitic bacteria. Under determined physicochemical
conditions ( large permanence time or high pH), these
crystals precipitate into incomplete forms such as
trapezoids, cruciform forms, prisms or into "fern leaf",
as the referenced case.
Urinary infections by ureolitic bacteria are
mainly caused by the genous Proteus, Morganella,

Las infecciones urinarias por bacterias
ureolíticas son causadas fundamentalmente por los
géneros Proteus, Morganella, Ureaplasma o
Corynebacterium urealyticum, con posibilidad de
cronificación. Se asocian generalmente con la
eliminación y/o formación de cálculos de estruvita y
carbonato de apatita. La ureasa procedente de los
microorganismos descompone la urea en dióxido de
carbono y amoníaco. El amoníaco es hidrolizado a
amonio y el dióxido de carbono a bicarbonato,
alcalinizando la orina. Esto moviliza los fosfatos
urinarios para intentar compensarlo, formándose
fosfato amónico que acaba captando magnesio y
calcio urinario libre.
La correcta identificación de las estructuras
cristalinas es fundamental para la orientación
diagnóstica, especialmente es este caso, ya que el
tiempo de demora entre la recolección de la muestra
y su análisis ha afectado a la calidad de la misma.
Sería recomendable notificar al médico peticionario la
probable aparición de los cristales in vitro debido a una
incorrecta manipulación preanalítica y la conveniencia
de valorar la solicitud de un nuevo sistemático y/o
urocultivo.
El hecho de que los autoanalizadores actuales
no sean capaces de clasificar adecuadamente dichas
formas cristalinas, unido a la posible confusión en su
identificación con cristales de origen farmacológico,
ponen de manifiesto la importancia de la observación
al microscopio óptico por personal debidamente
entrenado.
Garantizar las condiciones preanalíticas es
fundamental a la hora de determinar si un hallazgo
tiene relevancia clínica o es un artefacto.
Ureaplasma or Corynebacterium, with the possibility of
chronification. They are generally associated with the
elimination and/or formation of struvite calculi and
carbonate apatite. Urease from microorganisms
breaks down urea into carbon dioxide and ammonia.
Ammonia is hydrolysed to ammonium and carbon
dioxide to bicarbonate, alkalinizing the urine. This
mobilises the urinary phosphates in order to
compensate it, forming ammonium phosphate that
ends up capturing magnesium and free urinary
calcium.
The correct identification of the crystalline
structures is fundamental for the diagnostic
orientation; especially in this case, since the delay time
between the sample collection and its analysis has
affected its quality. We recommended to notify the
doctor the appearance of these crystals due to
incorrect preanalytical conditions, as well as the
evaluation of a new request for urianalysis and / or
urine culture.
The fact of that current auto-analysers are not
capable of appropriately classifying these crystalline
forms, together with the possible confusion in their
identification with crystals of pharmacological origin,
highlights the importance of observation under an
optical microscope by properly trained personnel.
Ensuring pre-analytic conditions is critical in
determining whether a finding is clinically relevant or
an artefact.

esfuerzos hasta hacerse de mínimos esfuerzos. En la
radiografía de tórax realizada al ingreso se observa
derrame pleural (DP) bilateral, por lo que es ingresado
para completar su estudio diferencial entre
insuficiencia cardíaca (IC) de novo o secundaria a AR.
Tras cuatro días de tratamiento deplectivo se realiza
radiografía de control donde se observa resolución
completa del DP derecho y persistencia del DP
izquierdo, por lo que se realiza toracocentesis
diagnóstica y evacuadora.
La muestra de LP recibida en el laboratorio era
de aspecto lechoso y se obtuvieron los siguientes
valores analíticos: pH 7,285, Colesterol 165 mg/dL,
Triglicéridos 16 mg/dL, Glucosa 64 mg/dL, Proteínas
5,25 g/dL, Lactato deshidrogenasa (LDH) 1056 U/L,
Adenosina desaminasa (ADA) 45 U/L, Hematíes
150/μL, Leucocitos 600/μL, Polimorfonucleares 85%,
Mononucleares 15%. Además, en la observación a
microscopio óptico se detecta la presencia de cristales
de colesterol (figura 1).Estos cristales se caracterizan
por sus diversas formas de presentación; como
pueden ser rectángulos, bastones, agujas, etc.
Además son incoloros, transparentes e irregulares.
Los cristales más característicos son los que
aparecen como láminas rectangulares o cuadradas
con una muesca en uno de sus ángulos y su tamaño
puede llegar a ser de hasta 100 μm de longitud (figura
1).
Los cristales de colesterol aparecen en el
pseudoquilotórax, forma poco frecuente de DP que se
caracteriza por su rico contenido en colesterol. Su
aspecto macroscópico es turbio o lechoso y no se
aclara tras la centrifugación, lo que permite
diferenciarlo del empiema. Sus características son un
alto contenido en colesterol (más de 200 mg/dL), cifras
bajas de triglicéridos (menos de 50 mg/dL) aunque en
ocasiones pueden ser altos (50-110 mg/dL), y
ausencia de quilomicrones. La presencia de cristales

The PF sample received in the laboratory was
milky in appearance with a Red blood cell count of
150/μL, a Total white blood cell count of 600/μL, with
85% polymorphonuclear leukocytes, and 15%
mononuclear cells. The biochemical analysis was as
follows: pH 7.285, Cholesterol 165 mg/dL,
Triglycerides 16 mg/dL, Glucose 64 mg/dL, Total
Proteins 5.25 g/dL, Lactate dehydrogenase (LDH)
1056 U/L, Adenosine deaminase (ADA) 45 U/L. In
addition, in the observation under an optical
microscope the presence of cholesterol crystals is
detected (figure 1). These crystals are characterized
by their various forms of presentation; such as
rectangles, rods, needles, etc. They are colorless,
transparent and irregular. The most characteristic
crystals are those that appear as large rectangular
plates (up to 100 μm in length) with a notch in one or
more corners (figure 1).
Cholesterol crystals appear in
pseudochylothrax, a rare form of PE that is
characterized by its high cholesterol content.
Macroscopically, its appearance is turbid or milky and
its lack of clearing after centrifugation makes it
possible to differentiate it from empyema. Its
characteristics are: elevated cholesterol content (>200
mg/dL), low triglyceride levels (<50 mg/dL) although
sometimes they can be high (50-110 mg/dL), and
absence of chylomicrons. The presence of cholesterol
crystals has diagnostic value, although it is not present

de colesterol tiene valor diagnóstico, aunque no está
presente en todos los casos de pseudoquilotórax. Es
causado principalmente por la tuberculosis, y en
menor medida por la AR. En el caso de esta última
solo el 3 – 5% de los pacientes lo desarrollan en algún
momento de su evolución (Canalejo Castrillero, 2005).
Se caracteriza por ser unilateral, produciéndose
comunmente en el lado izquierdo, es más frecuente
en el sexo masculino y se asocia a la presencia de
nódulos reumatoideos.
Tras la resolución del DP derecho y los
hallazgos encontrados en el análisis del laboratorio se
llega al diagnóstico de Síndrome de Contarini,
derrame pleural bilateral con etiologías distintas, en
este caso uno causado por una IC y otro consecuencia
de la AR que presenta el paciente.
Por ello cabe destacar la importancia del
laboratorio en el estudio de la etiología del DP, donde
los parámetros bioquímicos son fundamentales para
la diferenciación entre trasudados y exudados, y la
observación microscópica (figura 1) indispensable
para que estos hallazgos no pasen desapercibidos,
apoyando el diagnóstico correcto del paciente y
dándole un valor añadido al informe de laboratorio.
in all cases of pseudoquilothorax. Pseudochylothrax is
caused mainly by tuberculosis and to a lesser extent
by RA. In the latter case only 3 - 5% of patients develop
it at some point in their evolution (Canalejo Castrillero,
2005). It is usually unilateral, commonly occurring on
the left side, is more frequent in males and is
associated with the presence of rheumatoid nodules.
The right PE resolved after treatment and with
the aid of the laboratory analysis, the patient was
diagnosed with Contarini’s Syndrome. This syndrome
refers to the occurrence of bilateral pleural fluid
accumulation which can be explained by a different
cause for each side, in this case one caused by an IC
and another consequence of the RA presented by the
patient.
Therefore, it is important to highlight the
importance of the laboratory in the microscopic
examination of PE, where the biochemical parameters
are fundamental for the differentiation between
transudates and exudates, and the microscopic
observation (figure 1) indispensable so that these
findings do not go unnoticed, supporting the correct
diagnosis of the patient and giving added value to the
laboratory report.

Presentamos un caso clínico de una mujer
embarazada, mediante FIV, de 40 años de edad,
remitida a la Unidad de Genética Clínica del Servicio
de Análisis Clínicos y Bioquímica Clínica del Complejo
Hospitalario Universitario Insular Materno Infantil de
Las Palmas de Gran Canaria, debido a la edad
avanzada de la gestante.
We report a clinical case of a pregnant woman,
through IVF, of 40 years old referred to the Clinical
Genetics Unit of the Clinical Analysis and Clinical
Biochemistry Service of Maternal and Child Insular
University Hospital Complex of Las Palmas de Gran
Canaria, due to the pregnant woman´s advanced age.
A cytogenetic study is carried out for prenatal
diagnosis from foetal cell cultures from amniotic fluid.

Se realiza estudio citogenético para diagnóstico
prenatal a partir de cultivos de células fetales
procedentes de líquido amniótico. En las metafases
analizadas, con una resolución de 400 bandas
(bandas G), se observa una translocación recíproca
aparentemente balanceada entre el brazo largo de un
cromosoma del par 4 y el brazo largo de un
cromosoma del par 11. Una vez analizados los
cariotipos de ambos progenitores, se confirma la
translocación de novo: 46, XX, t(4;11)(q35;q21)dn
(figura 1).
Ya que no se observan malformaciones en el
seguimiento ecográfico, se decide continuar con el
embarazo asumiendo un riesgo medio empírico del 6-
10% de producir anomalías o déficits funcionales
significativos en el feto1.
La exploración al nacimiento objetiva la
presencia de un síndrome hipotónico y múltiples
rasgos dismórficos acompañados de miocardiopatía
no compactada. Se decide investigar en mayor
profundidad los puntos de corte de ambos
cromosomas translocados (4 y 11) mediante una
aproximación de Hibridación Genómica Comparada
(array-CGH). Se elige una plataforma de genoma
completo (180K+SNP, Agilent) con una resolución
media de 80Kb y de 20Kb en regiones diana.
Siguiendo la formulación ISCN-2016 se
identifican las siguientes anomalías (figura 2):
4q31.3(152202461_153962926)x1,
4q33q34.3(171838421_178341384)x1.
In the metaphases analyzed, with a resolution of 400
bands (G bands), a seemingly balanced reciprocal
translocation is observed between the long arm on
chromosome 4 pair and on the long arm on
chromosome 11. Once the karyotypes of both parents
were analysed, de novo translocation was confirmed:
46, XX, t (4; 11) (q35; q21)dn (figure 1).
Since no malformations are observed in the
ultrasound follow-up, it is decided to continue the
pregnancy assuming an average empirical risk of 6-
10% of producing significant functional abnormalities
or deficits in the foetus1.
In the initial exploration after birth, it was
detected the presence of a hypotonic syndrome and
multiple dysmorphic features accompanied by non-
compacted cardiomyopathy. To investigate in greater
depth the cut-off points of both translocated
chromosomes (4 and 11) by means of a Comparative
Genomic Hybridization (array-CGH) approach is
decided. A complete genome platform (180K + SNP,
Agilent) with an average resolution of 80Kb and 20Kb
in target regions was chosen.
Following the ISCN-2016 formulation, the
following anomalies were identified (figure 2:
4q31.3 (152202461_153962926) x1, 4q33q34.3
(171838421_178341384) x1.

La paciente queda diagnosticada de síndrome
de microdeleción del cromosoma 4. Tras búsqueda en
múltiples bases de datos clínicas se identifican varios
pacientes con similares microdeleciones; los
pacientes registrados presentan retraso mental
moderado (IQ 30-50), movimientos limitados de
extremidades, problemas de deglución, epilepsia, así
como otros rasgos y signos que comparten con
nuestra paciente2.
The patient was diagnosed of a Chromosome 4
Microdeletion Syndrome. After searching multiple
clinical databases, several patients with similar
microdeletions are identified; registered patients have
moderate mental retardation (IQ 30-50), limited limb
movements, swallowing problems, epilepsy as well as
other traits and signs that they share with our patient2.

Entre los genes contenidos en la microdeleción
destacan:
HAND2 (OMIM 602407): factor de
transcripción cardíaco. Su deleción se asocia
a las dismorfias faciales y a la cardiopatía no
compactada que presenta la paciente3.
GPM6A (OMIM 601275): producto proteico
específicamente expresado en neuronas4.
Algunas anomalías cromosómicas no son
suficientemente grandes como para ser visibles a
nivel citogenético. Las deleciones submicroscópicas
pueden contener a un solo gen o afectar a varios
genes muy próximos denominándose síndromes de
microdeleción o de genes contiguos. La mayoría de
los casos son esporádicos.
El array-CGH es una aproximación diagnóstica
muy potente, aunque no se usa de forma generalizada
en el ámbito prenatal. Ante situaciones clínicas
concretas y, siempre que sea realizado e interpretado
por personal experto, puede aportar datos muy
importantes en el diagnóstico y pronóstico de los
pacientes. Asimismo, es una herramienta vital para la
identificación de nuevas anomalías cromosómicas o
síndromes aún no descritos.
El caso descrito demuestra que, ante la
identificación de translocaciones recíprocas
aparentemente balanceadas, mediante cariotipo
prenatal (sean de novo o heredadas), queda
justificada la utilización del array-CGH como
herramienta complementaria. Este refinamiento
diagnóstico permite identificar la posible presencia de
alteraciones submicroscópicas, independientemente
de los hallazgos ecográficos. La información aportada
es crucial para un adecuado consejo genético y en la
toma de decisiones clínicas5.
Among the genes contained in the
microdeletion include:
HAND2 (OMIM 602407): cardiac transcription
factor. Its deletion is associated with facial
dysmorphic signs and non-compacted heart
disease also presented by our patient3.
GPM6A (OMIM 601275): protein product
specifically expressed in neurons4.
Some chromosomal abnormalities are not large
enough to be visible at the cytogenetic level.
Submicroscopic deletions can contain a single gene or
affect several very close genes called Microdeletion or
contiguous genes Syndromes. Most cases are
sporadic.
Array-CGH is a very powerful diagnostic
approach, although it is not widely used in the prenatal
setting. Faced with specific clinical situations and, as
long as it is performed and interpreted by expert
professionals, it can provide very important data in the
diagnosis and prognosis of patients. It is also a vital
tool for the identification of new chromosomal
abnormalities or syndromes not yet described.
The case described reveals that, given the
identification of apparently balanced reciprocal
translocations, by means of prenatal karyotype (de
novo or inherited), the use of array-CGH as a
complementary tool is justified. This diagnostic
refinement allows to identify the possible presence of
submicroscopic alterations, regardless of the
ultrasound findings. The information provided is crucial
for adequate genetic counselling and clinical decision
making5.

La Cistinuria es una hiperaminoaciduria debida
a un transporte defectuoso de cistina y aminoácidos
dibásicos (ornitina, arginina y lisina) en la membrana
apical del epitelio intestinal y del túbulo proximal renal,
lo que conlleva a una eliminación en exceso de cistina
a través de la orina. Se transmite con un patrón de
herencia autosómico recesivo, con una prevalencia
promedio en neonatos de 1/7000.
La única manifestación clínica conocida de la
Cistinuria es la litiasis renal de cistina, la cual
constituye uno de los tipos de enfermedad litiásica
renal más infrecuentes, con una prevalencia entre el 1
y 3% del total de las litiasis, alcanzando el 5% de los
cálculos en niños.
El cálculo de cistina se forma debido a su
excesiva concentración en orina y a su gran
insolubilidad a pH ácido. Además de obstrucción, los
cálculos de cistina resultan en importantes infecciones
urinarias con piuria, hematuria, cólico nefrítico y dolor
lumbar. En casos raros puede progresar a una
insuficiencia renal.
El procedimiento diagnóstico más sencillo es la
visualización directa en el sedimento urinario de los
cristales hexagonales planos y trasparentes típicos de
cistina, aunque solamente se detectan en la orina
entre el 19-26% de los pacientes homocigotos. Son
mas fácilmente observables en la orina de la primera
orina de la mañana, ya que ésta es más ácida y
concentrada.
El caso que se nos presenta trata de una
Cistinuria descubierta en un examen de orina
ordinario, en una paciente de 35 años sin
antecedentes de litiasis renal, que acaba de dar a luz
hace menos de 6 meses, por lo que su diagnóstico es
importante para el seguimiento del recién nacido. La
falta de sintomatología litiásica resulta sorprendente
dado las características de los cristales encontrados.
Cystinuria is a hyperaminoaciduria due to a
defective transport of cystine and dibasic aminoacids
(ornithine, arginine and lysine) in the apical membrane
of the intestinal epithelium and the renal proximal
tubule, which leads to an excess removal of cystine
through urine. It is transmitted with an autosomal
recessive inheritance pattern, with an average
prevalence in neonates of 1/7000.
The only known clinical manifestation of
Cystinuria is renal cystine lithiasis, which constitutes
one of the most infrequent types of renal lithiasis, with
a prevalence between 1 and 3% of the total lithiasis,
reaching 5% of the stones in children.
Cystine calculus is formed due to its excessive
concentration in urine and its high insolubility at acid
pH. In addition to obstruction, cystine stones result in
significant urinary infections with pyuria, hematuria,
nephritic colic and flank pain. In rare cases it can
progress to renal failure.
The simplest diagnostic procedure is the direct
visualization in the urinary sediment of the flat and
transparent hexagonal crystals typical of cystine,
although only 19-26% of homozygous patients are
detected in the urine. They are more easily observable
in the urine of the first urine in the morning, since this
is more acid and more concentrated.
The case presented is a cystinuria discovered in
an ordinary urine test, in a 35-year-old patient without
history of renal lithiasis, who has given birth 6 months
ago, so her diagnosis is important for newborn follow-
up. The lack of lithiasic symptomatology is surprising
given the characteristics of the crystals found.
The pH 5 urine shows hexagonal prism crystals
with six equal faces (Figure 1). The six axes and
planes of symmetry provide cystine a high capacity of
maclation, whose presence constitutes an excellent
tool that can be used in the prediction of lithiasic risk.

La orina de pH 5 muestra unos cristales con
forma de prisma hexagonal de 6 caras iguales (Figura
1). Los seis ejes y planos de simetría proporcionan a
la cistina una elevada capacidad y facilidad para
maclarse, cuya presencia constituye una excelente
herramienta que puede usarse en la predicción de
riesgo litiásico.
Existen 3 tipos de maclación que pueden
observarse en la figura 2:
1. Maclación escalonada: consiste en el
crecimiento telescópico sobre el eje vertical
de simetría de nuevos cristales.
Generalmente uno o varios lados quedan
borrados, lo que le da un aspecto
característico e inconfundible (figura 2A)
2. Maclación lateral: consiste en la formación de
nuevos cristales a partir de cualquier eje
horizontal. La figura obtenida es un
crecimiento en superficie, al estilo “panal de
abejas” en la cual los hexágonos comparten
una o mas caras (figura 2B)
3. Maclación mixta: consiste en el crecimiento
de nuevos cristales a partir de los ejes
verticales y horizontales. Son difíciles de
encontrar porque al formar masas de gran
tamaño son fracturados fácilmente durante la
centrifugación. La figura geométrica es
compleja y resulta de la combinación de los
anteriores tipos de maclación (figura 2C). Su
presencia indica un alto riesgo litogénico.
There are 3 types of maclation which can be
seen in figure 2:
1. Stepping maclation: consists of the telescopic
growth on the vertical axis of symmetry of new
crystals. Usually one or more sides are
erased, which gives it a distinctive and
unmistakable appearance (figure 2A)
2. Lateral maclation: consists of the formation of
new crystals from any horizontal axis. The
figure obtained is a surface growth, in the style
of a “honeycomb” in which hexagons share
one or more faces (figure 2B)
3. Mixed maclation: consists in the growth of
new crystals from the vertical and horizontal
axes. They are difficult to find because they
form large masses and are easily fractured
during centrifugation. The geometric figure is
complex and results from the combination of
the previous types of maclation (Figure 2C).
Its presence indicates a high lithogenic risk.

En la práctica clínica diaria podemos encontrar
aspectos fenotípicos en pacientes cistinúricos que no
siguen un patrón de conducta esperado, como en el
caso presentado, no todos los homocigotos
desarrollan clínica litiásica, el debut puede
presentarse desde la infancia a la edad senil, o no
presentarse. Incluso puede ocurrir que entre
hermanos homocigotos unos desarrollen litiasis renal
y otros no, siendo los factores medioambientales,
dieta y metabolismo semejantes.
En este caso el diagnóstico es importante, tanto
para el seguimiento clínico de la paciente, cuyos
cristales encontrados poseen un riesgo litogénico muy
elevado, como para la puesta en marcha de medidas
de prevención del recién nacido, entre los que se
encuentran alta ingesta de líquidos, alcalinización de
la orina y uso de agentes quelantes. Igualmente es
necesario llevar un control periódico de los niveles de
cistina en orina para ajustar las dosis del tratamiento
y evaluar la respuesta al mismo.
In daily clinical practice we can find phenotypic
aspects in cystinuric patients who do not follow an
expected pattern of behavior, as in the case
presented, not all homozygotes develop clinical
lithiasis , the debut may occur from infancy to senile
age, or not present. It may even happen that among
homozygous siblings some develop renal lithiasis and
others do not, the environmental factors, diet and
similar metabolism.
In this case the diagnosis is important, both for
the clinical follow-up of the patient, whose crystals
found have a very high lithogenic risk, and for the
implementation of prevention measures for the
newborn, among which are high fluid intake , urine
alkalization and use chelating agents. It is also
necessary a regular check of the cystine levels in urine
to adjust the doses of the treatment and evaluate the
response to it.
