Highly elevated liver enzymes in a child: case reportA male child developed highly elevated liver enzymes during

treatment with gabapentin. The child [age not stated] had high-risk neuroblastoma. He

underwent immunotherapy with the anti-GD2 monoclonalantibody. Three days prior to each cycle of immunotherapy, hestarted receiving oral gabapentin [dosage not stated]. Thegabapentin was continued until the end of eachimmunotherapy course. His AST and ALT levels increased to285 U/L and 312 U/L, respectively, during his first 3 courses[duration of treatment to reaction onset not clearly stated].Before the start of his fourth course, after 3 days of gabapentinpremedication, he had an increase in his AST level to 502 U/Land his ALT level to 500 U/L.

Gabapentin was withdrawn. The child’s laboratory valuesgradually improved.

Author comment: "A Naranjo score was calculated todetermine a causality-relation. A highly probable relationscore between use of gabapentin and high increase in hepaticenzymes was obtained."Bauters T, et al.
